Probationary Adoption Agreement: 1 Week Follow-up Report

Your written report is critical to the long term success of your adoption. We are here to support you and your adopted bird(s) for the long run! Please help us by complying with the terms of your Adoption Agreement. Failure to provide The Gabriel Foundation with these follow up reports may jeopardize the Permanent Adoption of the bird.

Diet- detail what you are feeding, when and how you are feeding your bird and what your bird is eating.*

A Typical Day – describe your schedule, your bird’s schedule and what activities you, your bird (and family or friends) do together.*

Exercise, physical and mental stimulation – what you provide, what bird prefers and why, bathing and shower preferences and schedule, and other relevant information*

Vocalization – when, how long, surrounding activity level and reinforcement*

Housing – cage size, bird’s time in and out of a cage, cage enrichment, cage location, perches, food dishes, etc.*

Bird’s sleep time and routine – what time, where, cages, location and pre-bed routine*

Socialization – describe how your bird interacts with you, your family, and/or friends, other birds and animals*

Are there any behavioral challenges you need help with? (Undesired vocalization, lunging, biting, fearfulness, etc.) No question is too silly for us. Remember that we are here to help and support you and your bird.*
